We use ultrasound or sonography in almost every aspect of our practice of Obstetrics and Gynecology. Most commonly, ultrasound is used to identify early pregnancy, and to rule out more serious problems. Later, it is used to exam the fetus in all stages of development. Examination of the fetal anatomy, position of the fetus, placenta and measurement of the fetal size are very common. Fetal well-being is also confirmed by evidence of normal amniotic fluid, fetal movement and fetal breathing movements. Also the number of babies is also pretty exciting (I still remember seeing my first set of triplets on ultrasound. Their mom was pretty excited also.)
Ultrasound is also very important for gynecology as well. We use ultrasound to examine the uterus, tubes, and ovaries, as well as the whole pelvic region. We can visualize the interior of the uterus, looking for fibroids, polyps and thickening of the lining of the uterus which we call the endometrium. Cysts of the ovaries are easily examined for size, character, and relationship to other structures in the pelvis. One of the most important uses for ultrasound in the pelvis is the ability to exclude ectopic pregnancy when a women has pain and bleeding in early pregnancy. |

| Infertility Testing and Treatment | ||||||||||||||||||||||||||||||||||
| There are many areas of fertility testing that can be accomplished very simply before going further with more involved treatment. We confirm ovulation, test cervical mucous, check for tubal patency, and check for endometrial or uterine problems before we go further to ovulation induction (fertility drugs), or insemination. The male factor is evaluated by a very simple semen analysis, done at the laboratory. We can do cup or intrauterine inseminations as necessary.

| Cancer Screening | ||||||||||||||||||||||||||||||||||
| The pap smear is still the most important cancer screening test and we encourage every woman to have regular paps. Endometrial biopsy is done in the office to rule out cancer of the uterus, and is used in conjunction with ultrasound to evaluate the endometrium. Ultrasound of the ovaries is not a screening test for cancer of the ovary, but it is very good for reassuring us that the ovaries are small and normal. The mammogram should be done at ages 35, 40 and then every 2 years after age 40, and yearly after age 50. | ||||||||||||||||||||||||||||||||||

| Approaching menopause can be a very difficult time for many women. Hormonal changes, added health risks, and changes in her physical body make this time extremely stressful. | The one service that our office provides of which we can be the most proud is the pelvic floor training or bladder rehabilitation service. This also utilizes biofeedback to accomplish amazing results without surgery. This service is headed by Audrey Cochran, who is certainly the foremost authority in bladder incontinence training in the southern SanJoaquin valley. It is not easy work, but for a motivated individual results can be dramatic. There are, of course, cases where surgical treatment is necessary, but surgery is still the last resort. Audrey teaches classes at CSUB, Bakersfield College, and in the community and is well known in the community for her care and interest in women's health care issues in later life.

For many women, excessive menstrual bleeding due to fibroids and other gynecologic disorders is a problem that causes them to miss as much as a month of work a year. During a new exciting procedure, hydrothermablation or HTA, doctors first use a scope to see the problem, then they use the same sheath to cleanse the body. To look inside a woman's uterus with a hystroscope is a relatively simple, extremely safe procedure and the only difference with the HTA part of the procedure is that in addition to looking inside of the uterus, a fluid is circulated through it. In the past, more traditional procedures such as a hysterectomy or a laser, which focused on clearing the uterine lining, were used but these procedures were associated with a certain degree of risk. That's why until now, many women shied away. With this procedure, a new confidence washes over patients as sterile saline fills the uterine cavity. It's room temperature in the beginning, then slowly warmed until it reaches ablation level, the heat continues for 10 minutes. Then cool saline is released to flush out the heated fluid. The ovaries continue to function after the procedure, a woman continues to ovulate, that's why women of childbearing age can take advantage of HTA.
